Insurance & Billing Policy

At EyeCare Associates of Yukon, we participate with most major medical and vision insurance networks, including EyeMed and VSP.

To ensure a seamless experience, please review the following important information regarding your coverage:

Network Status: 

While we accept many major plans, our provider status may vary depending on your specific policy. We recommend verifying whether our practice is in-network or out-of-network for your particular plan prior to your appointment. For example, we are providers for Blue Cross Blue Shield, but we are out-of-network for HMO plans.

Referrals: 

It is the patient’s responsibility to determine if their insurance plan requires a referral from a primary care physician (PCP). If a referral is required, please ensure it is obtained before your visit.

Patients using insurance:

Patients are ultimately responsible for any balances, co-pays, or fees not covered or denied by their insurance provider. Please note that this policy applies to all rendered services, including but not limited to contact lens evaluations and Optos retinal imaging, where applicable.

For Self-Pay / Private Pay Patients:

Patients without insurance coverage, or those choosing not to file through insurance, are responsible for the full payment of our standard self-pay rates at the time of service. Please note that this policy applies to all rendered services, including but not limited to contact lens evaluations and Optos retinal imaging, where applicable.

We recommend reaching out to your insurance carrier for assistance. Please note that for privacy and security reasons, insurance providers typically will not discuss policy information with us unless there is an active claim currently underway.

To help us ensure accurate billing and secure your medical records, we kindly ask that you bring your current medical insurance card and a valid photo ID to each visit.
